You may download a copy of the application form (see link above). Once downloaded, you can print the application from your computer

Submit an online application

All personal information is confidential and not shared with anyone.
Submit only one name per application.
Membership Dues: $35 per person for 2026!
Membership status becomes active once dues have been received by BMSC. You will receive a confirmation email once membership dues have been processed.
Include all names for membership when sending payment.

Brushy Mountain Shag Club Waiver and Release of Liability

Release: I understand that all Brushy Mountain Shag Club members and their guests participate voluntarily and at their own risk in all Brushy Mountain Shag Club activities. I release and hold the Brushy Mountain Shag Club and their respective officers, directors, employees, and agents harmless for any injury or damage to myself or my property resulting from, or in connection with any Brushy Mountain Shag Club activities or events. By signing the release, I certify that I have read this release and fully understand it and I am not relying on any statements or representations made by the Brushy Mountain Shag Club. I am also aware that by attendance at club functions I may be subjected to random or staged photography.

Activity Organizer: Brushy Mountain Shag Club (Sponsor).

Property Owners: Holiday Inn Express Wilkesboro.

In consideration of being allowed to participate in the Activity, IF member is INSURED participating in the Activity Member AGREES TO BE LEGALLY BOUND by the promises set forth below.

Member understands that these promises extend to the other members, officers, directors, agents, servants, employees, successors and/or assigns of the Sponsor, Property Owners.

Member WILL NOT SUE the Sponsor or Property Owners.

Members WILL FOREVER RELEASE the Sponsor and/or Property Owners from all liability for the injury.

Members WILL INDEMNIFY and HOLD HARMLESS the Sponsor and/or Property Owners from any loss, liability, damage, or cost of any kind that may occur as a result of the injury.

Member AGREES THAT THE ABOVE PROMISES ARE LEGALLY BINDING even if member contends that his/her injures are wholly or partly the result of NEGLIGENCE or other CONDUCT on the part of the Sponsor Property Owner for which a release is not contrary to public policy.

Member voluntarily ASSUMES THE RISK OF INJURY OR EVEN DEATH while participating in the activity.

Member recognizes his/her responsibility to INSPECT facilities, to exercise GOOD JUDGEMENT to ACT RESPONSIBLY and to OBEY all the Sponsor’s oral or written guidance, instructions (including signage) and warnings.

Member has read and understands these promises and is VOLUNTARILY signing the Brushy Mountain Shag Club Membership application.
